摘要:
目的：探究不同造影剂注射速率对多层螺旋CT直接法髂静脉成像质量的影响，以寻求最佳的注射速率。资料与方法：我们于2015年9月至2017年1月间将怀疑有髂静脉疾病的患者150例随机分为5组，并分别以注射速率为1.5 mL/s、2.0 mL/s、2.5 mL/s、3.0 mL/s、3.5 mL/s进行髂静脉CT成像。原始数据经过CT图像后处理后对每例血管显影质量进行评分，从图像的显影质量、对比度、有无"边流现象"及能否对疾病做出正确诊断进行评价。测量各评分组内髂外静脉的CT值并计算均值，对各评分组的结果进行统计学分析。结果：速率为2.5 mL/s组与3.0 mL/s组比较，差异无统计学意义；而2.5 mL/s组及3.0 mL/s组分别与其余三组进行两两比较时，差异均有统计学意义；1.5 mL/s组、2.0 mL/s组及3.5 mL/s组进行两两比较，调整显著性均大于0.05，差异无统计学意义。各评分组内髂静脉的CT值均数方差分析结果P<0.05，1-2、1-3、1-4、2-3、2-4、3-4各对比组次结果q值分别为4.89、4.94、5.30、4.35、5.02、4.67。P均<0.05，差异有统计学意义。髂静脉的CT值与各评分组的相关系数r值为0.87，P<0.05。结论：综合考虑图像质量、患者的耐受性及人身安全等因素，速率为2.5 mL/s是多层螺旋CT直接法髂静脉成像的最佳速率。
Abstract:
Objective: To investigate the effect of different contrast media injection rates on the imaging quality of multi-slice spiral CT direct iliac vein imaging in order to find the optimal injection rate. Materials and Methods: Between September 2015 and January 2017, 150 patients with suspected iliac vein disease were randomly divided into 5 groups. And the iliac vein CT imaging was performed at the injection rate of 1.5 mL/s, 2 mL/s, 2.5 mL/s, 3 mL/s and 3.5 mL/s respectively. The original data were graded after CT image post-treatment for each patient. From the image development quality, contrast, with or without "edge flow phenomenon" and whether the disease can make the correct diagnosis of four aspects of the evaluation. The CT values of the external iliac vein in each scoring group were measured and the mean value was calculated. The results of each evaluation group were statistically analyzed. Results: The rate of 2.5 mL/s group and 3.0 mL/s group, the difference was not statistically significant, but compared with the other three groups, the difference was statistically significant; 1.5 mL/s group, 2.0 mL/s group and 3.5 mL/s were compared, the signiftcance of adjustment was greater than 0.05, the difference was not statistically significant. The mean CT values of iliac vein in each evaluation group were analyzed by variance analysis P<0.05, 1-2, 1-3, 1-4, 2-3, 2, 4, 3, 4 The results of the comparison group were 4.89, 4.94, 5.30, 4.35, 5.02, 4.67 respectively. P were <0.05, the difference was statistically significant. The correlation coefficient between the CT value of the iliac vein and the score group was 0.87, P<0.05. Conclusion Considering the image quality, patient's tolerance and personal safety and other factors, the rate of 2.5 mL/s is the best rate of multi-slice spiral CT direct iliac vein imaging.
