先天性支气管闭锁的MSCT表现

MSCT Manifestation of Congenital Bronchial Atresia

  • 摘要: 目的:分析先天性支气管闭锁的多层螺旋CT(MSCT)表现,以提高该病的诊断水平。方法:回顾性分析15例先天性支气管闭锁患者MSCT表现;全部病例采用GE公司螺旋CT(Lightspeed 64)行胸部扫描,原始图像传送至AW 4.4工作站进行图像后处理重组,观察病变的特点并结合文献复习。结果:全部病例显示支气管黏液栓的改变,发生在左肺6例,右肺9例;13例显示黏液栓周围有肺气肿;2例无肺气肿,11例粘液栓伴行肺动脉分支变细,所在的肺叶血管细小,分支减少,分布稀疏。结论:MSCT对先天性支气管气闭锁具有较高的诊断价值,能显示先天性支气管闭锁的黏液栓和周围肺气肿的典型征象,后处理多平面重组(MPR)图像及最大密度投影(MIP)可任意角度观察黏液栓的形态、位置及其与邻近支气管、肺血管的关系。结合薄层重建图像观察可提示支气管闭锁部位。

     

    Abstract: Objective: To analyze the MSCT manifestation of congenital bronchial atresia, in order to improve the diagnosis level of the disease. Methods: Retrospectively analyze the MSCT manifestation of 15 patients with congenital bronchial atresia, scan the chest of all the 15 patients with spiral CT(GE light-speed 64), and transmit the original images to the AW4.4 workstation for Post-processing and reconstruction. Observe the characteristics of the lesion combining with literature review. Results: The change of bronchial mucus plugs appear in all the 15 cases, six cases in left lungs, 9 cases in right lung, the emphysema around plugs occur in 13 cases, two cases didn't affect with emphysema, 11 cases present the following symptoms that the bronchovascular bundles become thinner, the numbers are reduced and the distribution become more sparse. Conclusion: MSCT can contribute high value to the diagnosis of congenital bronchial atresia, it can show the mucous plugs and the typical symptoms of emphysema around, and the position of congenital bronchial atresia can be located by observing the shape of the mucous plug and its relations to the bronchial tubes and blood vessels through reformation of post-possessing MPR and MIP images, combining with slice images.
