多发磨玻璃影为主要表现的COVID-19患者CT及临床特征分析

Chest CT and Clinical Characteristics of COVID-19 Patients with Multiple Ground Glass Shadows

  • 摘要: 目的:探讨表现为多发磨玻璃影(GGO)新型冠状病毒肺炎(COVID-19)患者的CT及临床特征,提高对该病的认识。方法:回顾性分析12例确诊为COVID-19患者的临床及CT资料,分析其临床特征和影像征象,同时对CT征象及相应的病理基础进行对照分析。结果:12例表现为多发GGO样COVID-19患者中,发热11例,咳嗽7例,病灶数量≤5者2例,病灶数量6~10者2例,数量>10者8例;累及上叶10例,中叶和舌叶10例,下叶11例,同时累及上中(舌叶)、下叶者9例;小叶间隔增厚11例;病灶伴实变7例;胸膜改变11例,其中胸膜增厚10例,叶间裂少量积液1例。所有患者未见纵隔、肺门淋巴结肿大。结论:表现为多发GGOCOVID-19患者的CT及临床表现有一定的特征性,结合患者流行病学史及实验室检查,有助于该病的诊断和鉴别诊断。

     

    Abstract: Objective:To discuss the CT and clinical characteristics of COVID-19 patients with multiple ground glass shadows (GGO), in order to improve the understanding of this disease. Methods:Clinical and CT imaging data of 12 COVID-19 patients were retrospectively analyzed, at the same time, the signs of the CT images and the corresponding pathological basis were analyzed. Results:Among the 12 COVID-19 patients with multiple GGO, there were 11 cases of fever,11 cases of cough, 2 cases with the number of lesions ≤ 5, 2 cases with the number of lesions ranging from 6 to 10, and 8 cases with the number of lesions >8; there were 10 cases involving the upper lobe, 10 cases involving the middle and tongue lobe, 11 cases involving the lower lobe, and 9 cases involving the upper, middle (tongue) and lower lobes; there were septal thickening in 11 cases; lesions with consolidation in 7 cases; pleural changes were observed in 11 cases, among which pleural thickening was observed in 10 cases and a small amount of effusion was observed in 1 case. No mediastinal or hilar lymph node enlargement was observed in all patients. Conclusion:It has some certain characteristics on CT and clinical manifestations of covid-19 patients with multiple ground glass shadows (GGO). Combining with epidemiology and laboratory examination findings can help make the diagnosis and differential diagnosis.
