Imaging Diagnosis of Nuchal-type Fibroma

  • 摘要: 目的:探讨项型纤维瘤的CT和MRI特征。方法:回顾性分析8例NTF的影像学表现,病变均经外科切除和病理证实,其中2例行CT平扫检查,6例行MRI扫描,2例同时行MRI增强扫描,分析NTF的临床表现和CT、MRI征象。结果:8例肿物位于骶尾部皮下,质韧;CT和MRI显示病灶局部皮肤增厚,皮下脂肪内见片状、条索状影;CT呈稍高密度,T1WI呈低信号,T1WI压脂呈低-等信号,T2WI压脂呈低信号,DWI呈等信号,内见交错脂肪样密度或信号影;形态不规则,边界不清,无明确包膜。2例行增强扫描,均呈轻度强化。结论:项型纤维瘤的MRI和CT表现具有特征性,结合病史、症状及体征,有助于项型纤维瘤的诊断。
    Abstract: Objective: To study the CT and MRI findings of nuchal-type fibromas. Methods: Eight cases of surgically resected and pathologically confirmed nuchal-type fibroma were retrospectively analyzed. Results: All lesions were firm and located in the subcutaneous tissue of the sacrococcygeal region. Upon CT and MRI, local skin thickening was observed . The lesions were ill-defined and had irregular morphology. Moreover, the lesions showed slightly high density on CT, low signal intensity on T1WI and T2WI, and isosignal on DWI. Enhancement scanning was performed in two patients, with one mild and one moderate enhancement. Conclusion: The CT and MRI findings of nuchal-type fibromas have some characteristic features. Combined with medical history and clinical manifestations, these can assist in diagnosis

    (a)骶尾部偏左侧臀部皮下肿块(箭头),大小约4.3 cm×3.6 cm,边界不清,形态不规则,CT值30~40 HU,密度略欠均匀,内见夹杂岛状脂肪样密度,局部皮肤增厚并呈宽基底样膨隆;(b)大体标本带皮瓣;(c)镜下见粗大的胶原纤维条束杂乱排列,散在少许纤维母细胞,局灶胶原条束间见神经束。

    (a)(b)骶尾部中央皮下病灶(箭头),大小约3.6 cm×3.0 cm,T1 WI呈略欠均匀高信号,内见条纹状等信号,T2 WI脂肪抑制序列呈均匀等信号,形态不规则,边界不清,病灶部分包绕尾骨,未见骨质破坏;(c)肿块与骨骼肌对比呈等信号;(d)(e)增强扫描可见病灶部分条片状渐进性轻度强化灶。
